Many men live with heavy, lower-sitting brows for years without ever connecting them to a treatable condition. The brow position changes gradually with age, and because the change happens slowly, it is easy to assume it is simply how things are now. But for a significant number of men, heavy brows are doing more than sitting low — they are pressing down on the eyelids and reducing the field of vision.
We asked Miss Lisa Jagan, Oculoplastic Surgeon at Clinica London, to explain why this happens, why it affects men differently to women, and what can be done about it. Miss Jagan sees this regularly in her clinic, with a particular focus on how brow and eyelid position affects vision and comfort in men.
Brow position is different in men and women from the outset. Men naturally have a lower, flatter brow position, sitting closer to the bony ridge of the eye socket. As we age, gravity and a gradual loss of supporting tissue cause the brow to descend further. In men this descent is typically most pronounced at the outer, lateral part of the brow.
As the brows drop, the skin and soft tissue beneath them have nowhere to go but down, settling onto the upper eyelids. Over time, many men notice a growing sense of heaviness above the eyes, or find their field of vision feels more restricted than it used to — even when they feel perfectly well otherwise.

For some men, heavy brows are simply something they've noticed. For others, the excess tissue genuinely affects how well they can see.

Some men also find they have been unconsciously raising their brows throughout the day to compensate, using the forehead muscles to lift the brow out of their line of sight. Over months or years, this can lead to persistent forehead tension and headaches — something many men don't connect to their brow position at all.

The surgical incision made to elevate the brow was made above the eyebrow hairs and is barely perceptible.
Depending on the assessment, treatment may involve upper blepharoplasty (removing excess skin from the eyelid itself), a brow lift (repositioning the brow to reduce pressure on the eyelid), or a combination of the two. In some men, the eyelid itself has also started to droop independently of the brow — a separate condition called ptosis. Miss Jagan will assess the eyelid, the brow and the levator muscle together, since in many men these are all contributing to the problem at the same time.
